The Ameritech PrizesPlus Sweepstakes will be conducted over o three (3) month period from November 17 1986.
through Februory 7. 1987 Four (4) monthly prize winners Will be selected for each of the monthly contests. At
the conclusion of the three (3) months, one Grand Prize. one First Prize, one Second Prize and one Third Prize
winner will be selected from all entries received from the ree monthly contests. The monthly contests schedule is:
First contest begins Monday November 17. 1986. ond ends on Saturday December 13. 1986.
Second contest begins on Sunday December 14 1986. and ends on Saturday January 10,.1987
Third contest begins Sunday January 11, 1987. ond ends on Saturday Februory 7. 1987.
HOW TO ENTER

PRIZE WINNER SELECTION
Monthly prize winners will be selected in random drawings conducted by Creative Services Group, an
independent judging agency
The first monthly drawing will be held on Monday December 29 1986 from all entries postmarked or
deposited by December 13, 1986. and received or PrizesPlus Sweepstakes Headquarters by Saturday
December 20. 1986.
The second monthly drawing will be held on Monday Jonuary 26 1987. fromoll entries postmarked or
deposited between December 14, 1986. and January 10. 1987. and received at Sweepstakes Headquarters
between December 21. 1986, and January 17 1987
The third monthly drawing will be held on Monday March 2. 1987 from all entries postmarked or deposited
between January 11, 1987. and February 7 1987. and received at Sweepstakes Headquarters between
January 18. 1987 and February 14. 1987.
The Grand. First, Second and Third Prize winners will be selected in a random drawing held on Monday
March 2. 1987 from entries received at Sweepstakes Headquarters by February 14, 1987.
To be eligible as a selected winner the drawn entry form must include the correct solutions to a monthly
puzzle. All random drawings will be conducted by Creative Services Group, an independent judging agency
whose decisions are final on all matters relating to this promotion. Selected potential winners will be contacted
by phone within seven (7) days of selection to prequalify for eligibility They will then be notified of selection by
certified mail and will be required to submit documentoti'n of eligibility a signed and notarized affidavit to that
effect-ond to execute prize release and advertising consnt form. Failure to do so will result in disqualification.
Monthly prize winners will be eligible for the Grand, First Second and Third Prize drawings. All prize winners
must claim their prize within thirty (30) days of notificotiortor on alternate winner will be selected.
PRIZES
One Grand Prize: One prize of $10,000 cash or on escorted trip to the Orient for two people to Hong Kong.
Shanghai. Tokyo and other cities in the Orient to be determined by sponsor The trip includes round-trip coach
air fore first-class hotel accommodations for 17 days, 16 nights with most meals included. PLUS $1,000.00
spending money The trip must be taken between April 1 1987 and November 1, 1987. Retail value of th'e
trip and cash award is $9,550.00. The Grand Prize will be awarded to the selected winner who correctly solves
one (1) of the monthly contests and meets the conditions of the Sweepstakes.
