Dr. John Medders has some advice
for women under 30 who smoke and
wish to take birth control pills: if at all
possible, quit smoking.
Medders, resident in obstetrics and
gynecology at the University's Medical.
Center, said that while the Food and
Drug, Administration's (FDA) January
report concerning the risks of smoking
while on the Pill only indicated poten-
tial complications in the over-30 age
group, women under 30 should also be
aware of the potential risks of the com-
bination.
THE FDA'S REPORT was based on a
study that showed the combination
causes a dramatic increase in the risk
of strokes, heart attacks and other cir-
culatory disorders in women over 30,
with the greatest risk occurring in
women between the ages of 40 and 45.
According to the report, there are
between 3-4 million women in the
United States who smoke while on birth
control pills.
While stating there is as yet no scien-
tific evidence for the under-30 age
group, Medders said, "Women who are
taking birth control pills and smoke
should be apprised of the possible side
effects and remain alert for any sym-
ptomology related to the Pill
headaches, visual problems chest
pains, etc."
MEDDERS STRESSED the respon-
sibility of the physician in cautioning
the younger woman who wishes to con-
tinue smoking and begin taking the Pill.
"Her physician's obligation is to advise
her of potential risks based on her own
and her family's medical histories."
But, Medders pointed out, "the
ultimate choice to take the Pill is the
patient's."
The FDA's'report requires that the
following warning appear on birth con-
trol pill dispensers as of Aprfl 3:
"Cigarette smoking increases the risk
of serious adverse side effects on the
heart and blood vessels from oral con-
traceptive use. This risk increases with'
age and with heavy smoking (15 to 35
cigarettes per day) and is quite marked
in women over 35 years of age. Women

who use birth control pills should not
smoke."
Medders, one of two gynecologists
who run the Tuesday evening
Gynecology clinic at the University's
Health Service, said this warning will
appear on pills sold through the Health
Service pharmacy soon after the
FDA deadline. But, he added, "The
physician's responsibility to the patient
occurs long before the purchase of the
pills."
MEDDERS SAID while the decision
to prescribe the Pill should be in-

20 minutes of which are spent talking to
the patient - taking a personal and
family history and making sure the
patient understands the risks of the Pill
and how it works.
"I am willing to compromise with the
patient," he said, "as long as she un-
derstands the risks."
Medder's advice for the younger
woman who smokes and is taking the
Pill is first to quit smoking if at all
possible - "No matter what you say
about it, it's bad for you."

of any symptoms related to birth con-
trol pills which might occur,
"One of the first warning signs of
pending complications may be an in-
crease in blood pressure. This is one of
the reasons we only renew pills for one
year, so that the patient will have to
come back and see a physician."
Medders emphasized the importance
of communication between the patient
and doctor: "If you feel that you can't
communicate with your gynecologist.
think about changing doctors."
