QUESTION: Lately I've been getting hinsplints when I run. I've noticed it appens more often when I run on the ndoor track. Sometimes it gets so bad I ave to stop running for a whole week, nd then I feel like I lose a lot of ground n my conditioning program.\ I've only een running for about two months. Do ou know what causes this and how I an get rid of this problem? ANSWER: Shin splints are'ekprien- d as pain in the lower portion of-the eg, and are usually due to an inflam- ation of the tissues surrounding the hin bone. However, there are other ditions which may cause pain in the wer leg, and these have often been onfused with shin splints. Things such as disorders of the ascular system, which cause swelling f muscles and reduced blood supply to e area, and fatigue fractures, which are actual fractures of the shin bone, 4ve been misdiagnosed as shin splints some runners and other athletes. Since the latter are much more serious, - is important to make an accurate agnosis. In general, one indication is that if the pain subsides shortly after running, th re is a good chance it is shinsplints si ce these other conditions usually result in an increase in pain over time, even with resting. SHIN SPLINTS oc- ur more frequently in the early part of the training period, and, as you have experienced, more commonly on hard surfaces such as the indoor track or concrete sidewalks. Running too much on the toes or running with too much fot .and ankle motion can bring on insplints. ' If you develop shin splints, it is very portant to take care of them so that more serious injury does not occur. Ac- cording to Thomas Clark, a physician at University Health Service \who has an interest in sports medicine, rest is the most important treatment. This reduces the strain: placed upon the tissues and prevents swelling. Clark feels that if there is only h Service Hand moderate pain during or immediately after running, simply reducing speed and distance may allow it to resolve. However, if pain is severe and more persistent, complete rest from running is necessary. The amount of rest varies for each individual. You mentioned that a week of rest is sufficient for you. CLARK ALSO suggests applying ice right after running, and heat after a period of rest, to' the affected area. Also, taping the ankle if foot and ankle motion is painful, and taking aspirin, may help to improve symptoms and speed recovery. He emphasizes that if the pain has not resolved with rest in three weeks or if pain is very severe, a physician should be consulted to check for the other more serious causes of shin pain. You expressed concern that this period of rest that is necessary has caused you to "lose ground" in your conditioning program. Many beginning runners fear that if they interrupt their conditioning program by even one week, they will decline to their pre- conditioned state. However, a good thing to keep in mind is that it takes just about as long to fall out of conditioning as it does to get into conditioning. For you, this would be two whole months! IT HAS BECOME obvious that prevention is the key to eliminating shin splints. Some suggestions have been: " Gradual buildup of speed and distance to adjust to this new source of strain. Clark suggests that the runner learn to "read" the condition of his or her body each day to help avoid any ex- cess strain. Excess is, of course, relative to the person's condition, such as presence of illness, amount of fatigue, and amount of running he or she has become accustomed to. " Avoid running on hard surfaces. Grass, or other more resilient surfaces, are preferable. Although the uneven- ness of the grassy surface may cause other problems, such as twisting of the ankle or foot. " Always do stretching exercises both before and after running. * Wear shoes that fit well. Shoes that are too tight or too loose will interfere with the correct placement of the foot. Also, wear well-cushioned shoes, with good shock-absorbing qualities. * Have someone - preferably an ex- perienced runner or another with ex- pertise - watch you run to look for any changes that can be made in your run- ning style, such as eliminating toe run- ning. A podiatrist would be able to tell if you have "imbalanced feet" which might require an orthotic device - an angular-shaped leather, plastic, or book foam rubber shoe insert, available only by prescription, which helps the foot land flat and eliminates the twisting that causes leg problems. " Give yourself time to adjust to new surfaces, new shoes, environmental changes, and changes in training techniques. For instance, if you run a different route with more hills, decrease the speed and distance for awhile. " Do not run if you experience pain.
