The Sinai Guild is proud to initiate a FREE National
Women's Heart Study, and we can't do it without you!

Women's Heart Studies will take place at the following locations and dates:

•Oak Park Jewish Community Center, June 29
•Adat Shalom Synagogue, July 21

All locations will conduct
the Women's Heart Studies
from 8am -12pm

•Temple Israel, July 11 or August 4

•No appointment necessary

Petition Candidates: Other persons may be nominated for membership on the Board

of Directors by petition, signed by not less than twenty-five (25) members of the United
Jewish Foundation and filed with the Chief Executive Officer not less than 45 days prior
to the date of the Annual Meeting. Only one person may be nominated in each petition,
and no nominations shall be valid unless the nominee shall have consented to be a
candidate in writing, either in the petition or in a separate written document filed with the
Chief Executive Officer, not less than 45 days prior to the date of the Annual Meeting.
