nswer: Dear Stephen,
this is a large, relatively
complex, and still evolving
topic. Perhaps the best way to ap-
proach it is by discussing specific
hormonal activity.

Let's start with ghrelin (sounds
a little like "gremlin"), which
is sometimes referred to as the
"hunger hormone." Ghrelin is pro-
duced by fat cells in the lining of
the stomach and tends to increase
before meals and decrease after-
wards. One study has suggested
that ghrelin levels may be tied to
circadian rhythms, which may
-+ be flawed in obese people.
Other research suggests lack
of sleep produces ghrelin
and inhibits other hunger-
suppressing hormones,
resulting in weight gain.

One of those hunger-
suppressing hormones
is insulin, which appears
/ to play multiple roles in
the regulation of appe-
tite. A 2004 University of
Washington study suggests
that diminished insulin production
may be related to increases in the
levels of, and sensitivity to, ghre-
lin. Insulin-resistance, often linked
to the consumption of refined
carbohydrates, has long been cited
as a culprit in weight gain.
