The permanent teeth take up more space than the baby ones they replace, so even if the baby teeth look perfect in ideal alignment, there is probably going to be crowding. If the measurement is less than 22 millimeters, then you can guess there will not be enough room for the future permanent teeth. To determine if the palate of roof of the mouth is wide enough, the measure- ment you want to see between the right and left 1s1 permanent molars is 38 millimeters, or the length of a dental cotton roll. This dimension is fairly consistent from age 10 to late adolescence. Lastly, if your child can fit their little finger in between the upper and lower front teeth when the mouth is closed, then you may have a future problem with overlap. Also if you do not see at least 50% of the lower teeth when the teeth are closed together, there may be a problem with a deep over bite. All these are best evaluated by an orthodontist who can best determine growth direction and when to intervene.
