port services. Says Phyllis Schwartz,
JFS director of senior services, "We
have a team of professionals to help
families develop a plan of care, includ-
ing referrals for additional services and
private sources.
"We link you with Meals on
Wheels, transportation, home care
and personal care services, and respite.
When you leave for that well-deserved
vacation, we can make arrangements
so that your parent is contacted daily,
or taken to appointments, or given
meals while you're gone."
Care Connection, a care manage-
ment program for older adults who
need assistance managing their daily
activities, is a major focus of JFS. "A
customized care plan is developed for
each client," says Diane Sasson, pro-
gram director.
JFS' HomeShare helps the elderly
remain at home by matching a home
provider with a home seeker. "The
home seeker may provide some servic-
es in exchange for some of the rent,"
says Program Director Becky Eizen.
"It allows seniors to remain home and
maintain their independence, and it
brings affordable living to the home
seeker."
