High Dose
Inhaler Alert

Asthma sufferers who rely on
high doses of inhaled steroids
may be increasing their risk of
developing ocular hypertension
(glaucoma).
Researchers at McGill Uni-
/-)
versity Health Centre in Mon-
treal studied the relationship
between the use of inhaled and
nasal steroids and glaucoma in
48,118 patients. Their findings
were reported in a recent issue
of (beginital)The Journal of the
American Medical Association.

Patients who used high-dose
oral inhalers with 1,500 micro-
grams of flunisolide or 1,600 mi-
crograms of other inhaled steroids
for three months or longer had a
44 percent greater risk of devel-
oping glaucoma than nonusers.
An elevated risk of glaucoma was
not associated with nasal steroids,
which are typically administered
in smaller doses and often only
prescribed to treat seasonal al-
lergies.
Inhaled steroids have become
a common treatment for asthma
because they are an efficient way
to administer the anti-inflam-
matory agents that keep asthma
symptoms at bay.
However, the article's authors
warn that physicians should be
alert "to the possibility that in-
haled steroids may cause ocular
hypertension and open-angle
glaucoma" and monitor patients
who must take these drugs.

Estrogen And
Bone Density

Many joint-replacement
surgery patients are women suf-
fering from osteoporosis. But
women who are on long-term
hormone replacement therapy
are less likely to need such dras-
tic measures.
Researchers at the Universi-
ty of California San Diego stud-
ied post-menopausal women to
determine how effective contin-
uous estrogen replacement ther-
apy is in maintaining bone
mineral density (BMD).
Women who started taking es-
trogen during menopause - and
continued the treatment - had
the best BMD of all, but those
who started on estrogen after 60
had similarly high bone density.
Women who started using es-
trogen within two years of
menopause, but stopped after an
average of 10 years, had just
slightly better bone density than
women who never took estrogen.
The key is continuous estro-
gen use. Once women stop tak-
ing the hormone, they lose its
bone-strengthening benefits.
