"First we perform microsurgery
on the epididymis, under very
high magnification," explains Pro-
fessor Laufer. 'The epididymis re-
sembles a strand of human hair,
coiled at the side of the testicle,
and it's here the sperm cells flow
from the testis to mature. Our
aim is to find the obstruction
along its coils, get the sperm clus-
tered behind the blockage and ex-
- :Tact them."
Two of the seven couples in
whom there were no apparent
sperm production have become
pregnant following this procedure.
In one, the husband is a cystic fi-
brosis carrier and has no exit
tubes for his sperm. He lay anaes-
thetized in one operating room
during his epididymal aspiration.
His wife was prepared in moth-

/-er. When viable sperm cells were
found, Professor Laufer extract-
ed her ova for in vitro fertilization
A normal pregnancy followed this
unconventional start, and the cou-
ple are now the happy parents of
a little girl.
Even patients who appear to
have no sperm production at all
now have a chance of biologically
f athering children. 'We've found
,-)that in a majority of such patients,
we're able to find an essential few
sperm cells," says Professor
Laufer. "We biopsy the testicle,
remove a tiny slice of testis and
extract any sperm we find there.
Fertilization is then performed by
ICSI." ❑
