can Lung Association or Ameri-
can Cancer Society and
commiserate about quitting. His
approach combines hundreds of
different techniques, like snap-
ping a rubber band with every
urge to smoke, or deep breathing
to emulate the sensation of smok-
ing. For losing weight, the rubber
band "zapping" technique is used
along with self-esteem enhance-
ment exercises, and diet and fit-
ness regimens.
With the growth of health-
maintenance organizations,
American Institute of Preventive
Medicine has found itself on the
cutting edge of huge changes in
health-care delivery. Revenues
continue to climb and are now in
the $3-million range, Dr. Powell
said.
"A more up-and-coming area is
what is referred to as demand
management: Trying to reduce
health-care costs for companies
and HMOs by encouraging con-
sumers to make better decisions,"
he explained.
The cost of over-utilization of
the health-care system is astro-
nomical. Citing statistics from the
American Hospital Association,
Dr. Powell said 25 percent of all
doctor visits and 55 percent of all
emergency-room visits are un-
necessary.
"The self-care book helps some-
body make a more informed de-
cision as to what level of care they
should receive. It's not anti-med-
ical; it's appropriate care.
"In some cases, we're going to
increase utilization. Another
woman wrote and said that based
upon the self-care guide, she sus-
pected her son had a rare blood
disorder. She got him to the doc-
tor. It cuts both ways. The key
word is 'appropriate' use of the
health-care system. We're not tak-
ing away utilization that's neces-
sary," he said.
The Institute donated 1,000
child self-care guides to the De-
troit Department of Public
Health's Parenting Education
Program and has donated the
guides to an Indian reservation
in Canada and a homeless project
in San Francisco. ❑
