Name, Old Risk

The label—Syndrome X—might
have the high-tech echo of the
'90s, but the scenario is as old as
heart disease.
You put on a few extra pounds
that settle around the abdomen.
Blood pressure shoots up. So do
insulin and blood fats.
And pretty soon—especially if
you're a 40-to-55-year-old male—
you're a leading candidate for
coronary disease or adult-onset
diabetes.
"Syndrome X is a cardiovascu-
lar risk duster that's the best pre-
dictor for (heal-0 problems," said
Ralph LaForge, an exercise phys-
iologist at the San Diego Cardiac
Center.
Defined in 1988 by Dr. Geral
Reaven, a Stanford University en-
docrinologist, Syndrome X need
not be terminal.
Mr. LnForge, in fact, can write
you a prescription: 1,500 calories'
worth of exercise a week.
"You can manage the condition
with drugs. Or you can do it with
exercise, which is more cost-ef-
fective," he sad.
For the average man, the
bench mark 1,500 calories' worth
of exercise per week translates
into five or six hours of exercise.
"Sometimes we start them at
800 calories' worth of exercise and
build up," Mr. LaForge said.
Finding the time is no easy
task, especially for men with fam-
ilies and jobs that demand more
work in less time—a major source
of stress.
"That type of stress is probably
related to craving quick-fix calo-
ries," he said.
The calories often add up to the
extra weight in the abdomen.
"That's usually the first place
it goes on," he said. "Abdominal
fat tends to be more detrimental
to health than in the thigh and
arms. It's metabolically differ-
ent—it relates more to blood pres-
sure, cholesterol and elevated
insulin levels."
Combine all this with what is
known as the "hot reactive per-
sonality" and you've got a heart
attack waiting to happen.
Being reactive, or prone to
anger, can be one of the most
lethal forms of Type A behavior.
In this case, more of the mind-
body approach is suggested: ex-
ercises such as yoga or tai chi,
with enough walking or addi-
tional exercise to reach the calo-
rie-burning goals.
Another goal of the exercise is
to enhance the heart-health HDL
(high-density lipoprotein) choles-
terol. It should be above 45.
But two out of every 10 men
are unable to elevate their HDL
levels with exercise. The answer
in this canse: medication. ❑
