Physician Referral and
Information Service

Locates the Right Doctor for You

W

hen some people need a doctor they may seek the recom-
mendation of a family member or friends. However, we wel-
come you to contact SinaiSource physician referral service for a
physician who meets your needs or those of your family. The ser-
vice refers callers to more than 500 members of Sinai's active med-
ical staff in specialties from obstetrics/gynecology to cardiology,
orthopedic surgery and family practice.
When you call 1-800-248-3627, you will be asked to identify
which type of physician you need (Sinai's representatives can help
you do this), a preferred office location and your health insurance.
Feel free to discuss special requests such as a male or female physi-
cian. We will make every attempt to refer you directly to the type
of physician you've requested and make an appointment during
your call.
After calling Sinai's physician referral service, you will receive
additional information in the mail on the physicians including
their educational and training background, specialties, subspe-

cialties and office hours.

Sinai Health Care Foundation

Tn its third year of operation, the Sinai Health Care Foundation reaches thou-
-I-sands of contributors in the metropolitan area who wish to support Sinai Hos-
pital's tradition of medical excellence.
The Foundation raises and administers funds to meet the lialthcare needs of
the community. Thanks to many generous donors, the Foundation is me
mission of providing continued support to Michigan's on ly jewish_sponsored ho
pital. Richard Roth chairs the Foundation's Board of Trustees.

Sinai Speakers Bureau Program
Offers Community Education

Among the .Foundation s efforts are
Sponsors Club. Begun in 1993, this annual membershiP drive raised

more than $17,000 from 340 donors last year. All $50 membership gifts
are listed in the quarterly Foundation Fecitures.

Founding 500. This special campaign raised $220,000 from 440 donors,
each of whom contributed a minimum of $500. Their names are listed on
the Founding 500 Wall of Honor in the Hospital's West Outer Drive lob-
by.

Heritage Ball. The annual Heritage Ball raises funds fill' a particular
area of service. -The 1994 Boll drew more than 800 attendees, who con-
tributed more than $400,000 in support of Sinai's Radiation Oncology
section.

Endowment Funds. There are many giving opportunities for those who
wish to provide permanent funding for specific services or facilities. En-
dovvment gifts may also be made in conjunction with the . United Jewish
Foundation.

200 people, the Sinai Speakers Bureau
has something for everyone! From start-
ing an exercise program for a healthy
heart, to presentations on arthritis and
stress management, our program is tai-
lor-made just for you.
The Sinai Speakers Bureau is Sinai's
way of reinforcing the important role of
education in helping you to maintain a
healthy lifestyle.

Tribute Gifts. Tributes of $5 or more provide a meaningful way to ac-
