They in- clude 'enactment of a nation- al health law that would decentralize existing health- care agencies, particularly Kupat Holim, the Histadrut- affiliated giant with the largest membership. Histadrut, Israel's huge trade union federation, is a Labor Party stronghold. But the cautious reception the commission's proposal got from Mr. Shamir and Health Minister Ehud Olmert did not necessarily reflect reluctance by Likud to tangle with Labor. Rather, the Likud politi- cians may have imagined the public reaction to the new system, which would require it to pay for what it now receives free or for a small premium. The commission proposed replacing the Health Min- istry with a National Health Authority. It would separate existing health-care agen- cies into regional entities, which would operate in- dependently. Hospitals would be put on a profit-making basis and the private sector would become more involved in health insurance for profit. The idea, the commission said, is to improve the in- come of doctors and so end the constant round of strikes and work slowdowns which have paralyzed health care in Israel in recent years, leading to the commission's creation. But the changes would re- quire huge sums of money. The state would have to im- pose a health tax, similar to the monthly National In- surance levy. The health-care agencies would be authorized to charge patients on a per- visit basis, whereas . now payment of a single premium allows unlimited visits.
