from hematoma and skin
necrosis (tissue death), and
most pediatricians have only
limited experience with it.
Consequently, it appeared un-
til only a few months ago that
circumcision would soon
resume its status as the un-
mistakable mark of the Jew-
ish man.
But not so fast. The Amer-
ican Academy of Pediatrics
has just revised its official
recommendations, and the
debate is heating up again.
Not that the medical estab-
lishment has reverted to its
previous enthusiasm for cir-
cumcision as a universal
panacea, but parents are be-
ing urged today to consider
the potential medical benefits
as well as the inherent disad-
vantages and risks of circum-
cision.
According to Dr. Donald
Schiff, president of the Acad-
emy, doctors should neither
condemn nor actively encour-
age the operation for non-
Jewish infants, but they
should point out all the pros
and cons.
Urinary Tract Infection. In
1985, U.S. Army doctors
found that uncircumcised
males suffer from urinary
tract infections 10 times more
frequently than circumcised
male-s. Longitudinal studies
support that conclusion: as
circumcision has abated over
the past 10 years, the in-
cidence of urinary tract infec-
tion has increased in direct
proportion. The Academy
points out, however, that
"these studies in Army hospi-
tals are retrospective in
design and may have metho-
dologic flaws. For example,
they do not include all boys
born in any single cohort or
those treated as outpatients,
so the study population may
have been influenced by selec-
tion bias."
Phimosis. At birth, a
baby's foreskin is firmly at-
' tached to his penis; within the
first two years, however, the
layers begin to separate, un-
til the foreskin can be freely
retracted. In rare cases, the
foreskin remains tight as the
child approaches puberty, and
phimosis — the inability to
retract the foreskin — results.
In addition to causing severe
pain and hygienic problems,
phimosis interferes with erec-
tion and occasionally pre-
cludes it altogether. Obvious-
ly, circumcision prevents phi-
mosis entirely, just as it
excludes paraphimosis —
swelling from accumulated
fluids — and balanoposthitis
— inflammation of the fore-
skin and glans. When these
conditions afflict a mature
man, the remedy is circumci-
sion, and what might have
