One trend stands out clear- ly in the confusion: "Where Jews live in mixed popula- tion, they are consistently more at risk for IBD than are their neighbors." An IBD document from the Ileitis and Colitis Foundation was more specific about IBD, calling it "predominantly a disease of the developed world?' The foundation document listed the countries reporting the highest incidence rate of IBD as the United States, England, Scotland, the Scan- dinavian countries, a few countries in Western and Central Europe and Israel. The risk of developing IBD is about four to five times, greater among- Jews living in the United State and Europe, the foundation found, noting that rates have not been high among Israeli Jews. The disease is incurable at present, though drugs are us- ed in other countries to ease symptoms of the disease. The drugs are still awaiting Food and Drug Administration ap- proval for use in the United States. Some doctors prefer to ig- nore any Jewish factor in IBD. Dr. Steven James of the National Institute of Allergy and Infectious Diseases at the National Institute of Health argues that studies showing Jews with a disproportionate- ly high incidence of IBD were first done in -cities with large Jewish populations. Another factor, he says, is that many years ago, only persons with access to good medical care were able to have the diagnosis. Jews, who usually have bet- ter access to medical care in those urban areas where the studies were • made, "were diagnosed more often for that fact alone," he says. Mendeloff and his col- leagues were described as be- ing aware of these factors, but maintain that the disease is linked to Jewish genes. Last year, four doctors at the Ichilov Hospital and Sackler Faculty of Medicine at Tel Aviv University review- ed all the data and proposed a compromise. Their spokes- man, Dr. T. Gilat, said that IBD represents "the interac- tion of two sets of factors." The data offers strong sup- port for the conclusion that environmental factors are paramount in the causation of IBD, "but current research also suggests the coexistence of genetic factors!'
