The municipal fluorida- tion system is part of a pilot plan to improve dental health in Israel. "Fluoridation has been established for 34 years. As a public health technique, it is recognized from Texas to Tashkent." In the first years after the establishment of Is- rael there was no need for the introduction of such a dental health care sys- tem, Kelman noted. Be- fore Israel implemented the national water car- rier system, there was probably more fluoride in the water. "In the 1950s, the major- ity of Israel's water supply came from wells. It is a fact that the deeper the source of water the more fluoride is present," he said. Well water soaks up the fluoride embedded in natural mate- rials surrounding the well. Surface water, such as that from the Sea of Galilee, which is filled from Mt. Hermon, has little fluoride. Also in the 1950s, "there was not much tooth decay in Israel," he said. Sweets did not proliferate as they do today, and the sugar con- sumption pattern was different. "Americans had not yet taught the Israelis to eat candy," Kelman said. Accordingly, it was felt that additional fluoride was not needed. "Since then, 25 years have passed, the world has changed, and so has Israel. At best, the fluoride reaches 50 percent of that required." Avi Samuel, director of the Jerusalem water de- partment, said the op- timum level of fluorida- tion is .08 parts-per- million. At present in Jerusalem, .04 ppm are found naturally in the water, so another .04 must be added. The municipal fluorida- tion system has been in- stalled at Kikar Holland, on Mt. Herzl. The installation includes instruments to check the amount of fluoride by remote control. "The system is the most modern by world standards," Samuel said. When the system is com- pleted, 60 percent of Jerusalem's water will be fluoridated. "It's hard to say how long it will be before the project is completed," Samuel con- tinued, "because a private company, not the munici- pality, is doing the con- struction work. There is no reason why the installa- tion couldn't be working in September," Kelman noting that the project has been under discussion and in the planning stages for several years. "Nine years ago, as direc- tor of dental health for the Health Ministry, I was in- vited to take this position. When I arrived, discussions began. In 1974, the late Yit- zhak Sitton, a great pioneer in water development, who was head of the Jerusalem water department before Samuel, wrote to the minis- try to suggest fluoridation." Discussing the overall dental health picture in Israel, Dr. Kelman said, "I sus-pect that the amount of fluoride has declined, and coupling this with the overwhelm- ing consumption of sugar, has produced something like 17 million rotten teeth." As a result of the introduction of the system, there will be a 60-70 percent reduction of dental cavities, he said. Fluoride reduces tooth decay in all age groups, but is most effective in children. Until the age of 12, children are most exposed to tooth decay, because they have very little fluoride in their outer enamel." "Out of 30,000 children in Israel, there are 150,000 cavities. In five years after fluoridation, there will be 100,000 dental cavities in the same number of chil- dren, and in 10 years, 30,000 cavities. I would like to see Israel completely fluoridated in three-four years. It is not a financial problem, we have enough money for it," Kelman said. The Jerusalem municipal water fluoridation system costs about IL 700,000, (ab- out $23,300) which works out to about IL2-3 per capita (about $.10). The fluoride to be added to Jerusalem's water will be manufactured in Haifa. "There is absolutely no dif- ference between natural and •artificial fluoride," Kelman said. "This is an important point because people attribute a mystical significance to things natural. I know of no plant in • the world that uses natural fluoride," he said.
