The blanks are being made avail- able as the Social Security Admin- istration seeks to contact 3,100,000 senior citizens before a March 31 deadline imposed by the Medicare law. President Johnson has pro- claimed March as "National Medi- care Enrollment Month." He urged all federal agencies and all citizens to cooperate in enrolling senior citizens in the program . Everyone 65 and over is eligible for basic hospital benefits under Medicare. Senior citizens who reached 65 before 1966 face a two- year delay if they do not sign up by March 31.
